Ongoing nursing support for individuals living with chronic or complex health concerns, shaped by a proactive, long-term approach including regular in-home visits, medication support, symptom check-ins, and coordination of care to help maintain comfort, stability, and daily well-being.
Care Coordination & Advocacy
Navigating the healthcare system can feel overwhelming. I provide dedicated support to help coordinate your care across providers and settings — communicating directly with your physicians and specialists, attending appointments when needed, implementing medical recommendations, arranging equipment and supplies, and advocating for your needs every step of the way.
Fees are based on the scope, duration, and location of care, with visit structure tailored to your specific needs.
Initial private RN visits begin at $195, with bundled care options and specialty services available.
All rates are inclusive of Hawai’i General Excise Tax.
